Utah care guidebook
Bring the question you have, not a treatment decision.
You have arranged a conversation and worry you will forget what to ask. A short note can hold the place for you.
Put your concern in your own words
Write what you want help with and what it has been like for you. You can say you are curious about peptides without choosing a medicine in advance.
Make a note of current medicines, supplements and known allergies. Add questions about earlier treatment and any phrase you want the clinician to explain slowly.
Questions to leave space for
Leave room for an assessment
Utah assessment duties, checked September 28, 2026.
Before prescribing online, a Utah clinician must identify the health problem, review your relevant health history and check why the treatment might be unsuitable.
- What health problem does the assessment point to?
- What other approaches could address it?
- Why this exact medicine and this form of it?
- What would make you advise against this treatment?
Ask for a plan that survives the end of the visit
- What needs checking before a decision?
- What changes will tell us how the plan is working?
- What problems would need prompt help?
- Who answers questions between visits?
- How will my regular clinician receive the record?
- What are the full charges and cancellation terms?
Try describing the plan aloud before you leave. When wording is hard to follow, ask for a clearer written version. Keep unresolved questions on the sheet.
A small sheet to take with you
