Match the question to a record that can answer it.

You want to know whether a confident statement has evidence behind it. Use the kind of record that fits the claim.

Choose a record by the question you have

For the person, use the Utah record

DOPL, Utah’s professional licensing office, offers a search for health professionals and pharmacies. It also links a separate search for disciplinary actions, meaning formal actions against people or businesses it licenses.

A license record is the place to check a claimed name and profession. For a restriction or a license type you do not understand, ask the licensing office what that record means.

For online care, read the state’s duties

Utah’s law for online care explains what an assessment must include, what records to keep and what follow-up is required. It also limits relying on a questionnaire alone.

Bring a specific question to the text: what must happen before prescribing, or what happens after a visit? That is easier than trying to read a whole chapter as a beginner.

For a medicine, distinguish approval from preparation

FDA means Food and Drug Administration. For compounded medicine, FDA does not check before sale whether it is safe, made well or works as intended.

In FDA’s drug directory, the number that identifies the medicine is called its National Drug Code (NDC). A seller showing a code or listing still has not shown FDA approval.

Ask the clinician to show the approved product information or explain the preparation being proposed. Keep those answers distinct.

Dates mark the limits of an answer

The Utah evidence cited in this guide was checked September 28, 2026; the federal status and cost evidence was checked September 26, 2026.

Those dates mark the evidence used, not a live check of a person or an offer. Return to the official record when making a choice. If a claim and the record differ, ask for the difference to be explained.
